As a CCC employee or volunteer, I agree to practise the following:
Declaration: I understand and abide by the conditions in the above COVID-19 Agreement. I agree that I will not attend the Centre until this agreement has been signed and returned, and that my continued attendance at work relies upon this agreement being upheld.
- I will follow the direction of the current COVID-19 safe practices.
- I will wear an appropriate face mask whilst at work with others and to and from the workplace if required by CCC or mandated by the current Victorian Government.
- I will implement appropriate hand hygiene prior to entering the building and whilst in the building.
- It is my responsibility to be aware of and adhere to social distancing of 1.5 metres in the workplace where possible.
- I will clean and sanitise my work area on starting work, during my stay, and on leaving.
- I will not attend if I have any cold or flu symptoms such as a runny nose, sore throat, headache and /or cough and I will complete either a RAT or PCR test as required.
- I agree that the Manager or my Coordinator can direct me to leave work and go to be COVID-19 tested or go home if it is determined I am unwell during attendance at the Centre or experiencing any cold and flu like symptoms.
- I agree to immediately have a COVID-19 test if I have any cold or flu symptoms and isolate at home until I receive a negative test result or follow current DHHS advice if positive. 9. I agree to be fully vaccinated against COVID-19 as required by CCC Mandatory COVID-19 Vaccination policy.
